We do not "participate" with any
insurance companies at this time. Payment is expected at the time services
are rendered. We will provide you or
your insurance company with a completed HCFA
1500 insurance claim form with appropriate
diagnosis and procedure codes. You may
submit this form to your insurance company or
we will mail it for you. Many (but by no
means all) health insurance plans provide some
degree of reimbursement for “out of plan”
visits. When more information is
required, our staff will be pleased to work
with you and your insurance company.

A referral is not necessary but, with a
referral, your insurance plan may increase its
level of payment.
It is best to set up a new appointment
during normal business hours (see above) when
we have access to our computerized scheduler.
However, after hours scheduling may be
possible by calling the main office number.

Yes, the office building is one story and
our office is just off the parking lot.

For
any intravenous treatments, it is recommended
that you bring something to eat and drink as
well as reading or work material.
We do have TV VCR’s in our treatment
rooms so, if you have a favorite movie you
would like to view, please bring it with you.
Environmental
medicine is the diagnosis and treatment of
diseases that are directly or indirectly
caused or made worse by factors in our
environment.
These could include sensitivities to
foods, allergens, such as pollen, heavy metals
or other substances.
Integrative medicine is the holistic
combining of
modern day medical practice with more
traditional, complementary, and alternative
practice for the total well-being of an
individual.
It employs modern medical diagnostic
testing with traditional and non-traditional
treatments to improve overall well-being.
At
CEIM we believe that through careful
evaluation and truly listening to your history
and paying attention to your symptoms we can
often find causes that have been overlooked or
ignored by other health care providers.
We believe that each person we see at
CEIM is an active partner and, indeed, the
director of their health care.
Together, we can find the best
treatment to meet your needs.
Cost
for the initial visit, physical examination,
and record review is approximately $250.
There are additional charges for any
laboratory or other testing that is done.
These can vary greatly.
Any treatments or supplements are an
extra charge and also vary depending upon what
they are. All costs will be reviewed with you prior to our doing any
testing or treatments.
For
payments, we accept MasterCard, and VISA credit cards. We also accept
payment by personal check (imprinted only) or
cash.
We

In
a true medical emergency it is always best to

call emergency services.
For other concerns, call the main office
telephone number and you will promptly receive
a return call.
For
us to release patient's records we must have
written consent; standard copying fees apply.
We
do accept new patients. A routine
appointment is usually scheduled within one
week or sooner, depending on severity of their
condition.

call us at 865.934.0133 for any appointment
needs. After hours, you may leave any
non-urgent messages with the answering

Last minute refills are uncertain. To
insure safety and accuracy, we do not refill
prescriptions after regular office hours.
Note: each refill request is evaluated by our
professional staff. If necessary, the
physician may require an office visit to
determine the appropriateness of a refill
request.
Following
the HIPPA Privacy guidelines, all
laboratory test results will be communicated
to you in person or by a mailed medical report--unless
there is a life-threatening result which we
will notify you by the fastest means
possible. Ideally we like to gather all
test results and set up an appointment for you
to review the results (there is no charge for
the appointment to review lab results).
It may take several days or even a week for us
to receive all your results so please be
patient. You are welcome to call us at
anytime to check on the status of your lab
tests.
